Employment Screening

• Agreement for Service – Employment Screening (without credit reports)
  This form must be filled out completely and attachments added to start your account. Fax back the signed and dated form to our office as indicated in the page footer.
• Agreement for Service – Employment Screening (with credit reports)
  This form is required if you plan to pull employment-related credit with other employment products.  Clients should be aware that several states (including Washington) place significant restrictions on the use of credit reports for evaluating an individual's fitness for employment. These states may require special application and consent forms if credit is to be used.
• Employment Inquiry Release
  Pprospective employees should read, sign and date this form. The information on this form should match the application. Employers are to retain this form for compliance purposes.
• DOT Drug Screening Packet
  This packet consists of consents, checklists and disclosures for drivers covered under Title 49 regulations.
• Washington State MVR Form and Instructions
  This packet consists of a Washington State MVR form with Alliance listed as the recipient, and the Full Employment/Commercial Record box checked. Use this form to make Alliance your agent for the MVR. The form has interactive fields so that applicants can fill it out using a computer and then print it. Instructions for use are also included.

  This summary of rights under the FCRA to be provided to consumers.

Tenant Screening

• Agreement for Service – Private Landlord Tenant Screening
  This form is for property/apartment managers with fewer than 100 units. This form must be filled out completely and attachments added to start your account.   This form is for private landlords to provide to prospective tenants. Once a tenant fills out the information and signs it should be checked for accuracy and faxed to 800/289-9246.
• Commercial Property Management Rental Application (Generic)
  This form is for property managers/apartment managers to provide to prospective tenants. Once a tenant fills out the information and signs it should be checked for accuracy and faxed to 800/289-9246, or the information can be entered in online. Custom forms with your location's identifying information can be provided, along with form fields in the PDF so that prospective tenants can type in the information (call the office for more details).

Mortgage Credit Reporting

• Agreement for Service – Mortgage Credit Reports

• Letter of Intent – Mortgage Credit Reports
  This letter must be provided to Alliance 2020, on YOUR MORTGAGE COMPANY LETTERHEAD, stating the permissible purpose(s) for which you intend to use credit reports. It must be signed by the broker.
• Add a Branch Agreement (for Brokers to Add a Net Branch)
  This form includes an authorization from the parent company and a credit card authorization agreement for the branch. Has interactive form fields so the agreement can be filled out using Acrobat or Acrobat Reader.  Each branch office must also submit a complete Agreement for Service -- Mortgage Credit Reports to meet our compliance requirements.
